You must be a member of Monad Federal Credit Union before applying for access to Monad@Home. All requests must be made in writing using this form or a facsimile. You may mail or deliver the signed original to our office at 1817 W Sylvester in Pasco, Wa 99301. Your account will be activated within the 48-hour period following the business day in which we receive your written request. Access to Monad@Home will be controlled by means of passwords and extended security questions. This password may be any combination of numeric characters that you wish. When your account is first activated, you will be assigned a temporary password. It is your responsibility to change this password.
